ANYONE CAN MAKE A REFERRAL FOR HOME HOSPICE SERVICES.
Terminally ill persons residing in Grayson County may be referred to Home Hospice by phone call to
our Sherman office at 903.868.9315. Terminally ill persons
residing in Fannin County may be referred to Home Hospice by phone call to our
Fannin County office at 903.583.9320. Terminally
ill persons residing in Cooke County may be referred by phone
call to our Gainesville office at 940.665.9891. This call may be made by the patient's physician, family,
friends, hospital discharge planner, nursing home personnel, the
patient, or anyone interested in the patient's care and
condition.
After a referral is made, our Patient Care Coordinator contacts
the patient's attending physician and then the patient and family
to explain services.
It is best if a referral is made with full knowledge of the
patient and/or his/her family.
