By referring appropriate patients to Home Hospice, you offer an additional array of supports that not only address the patient’s acute medical needs, but surround them and their families with essential services to help through the last days of life.
You remain the physician of record, while our professional team coordinates critical care with you. By implementing your directives, patients are reassured that their relationship with you remains intact, while our caring staff implement best care in the residential setting.
Knowing that your patients are receiving exemplary care and that family members are being given support and guidance, at a time when their comfort is a priority offers reassurance to many dedicated physicians. We also answer questions posed by the family and help link them to needed community resources, so that they have a variety of important supports.
